Deaths

If one of your employees dies in service, use the form below to tell us.

Notification of death (Word doc, 85 KB) 

The form will help us to determine:

• The beneficiaries of the death grant
• Whether any survivor pensions are payable

Please check the parts completed by the member's representatives. For example, are the account details in the name of the beneficiary or a joint account which bears their name?

Forms should be emailed to spfo@glasgow.gov.uk using the Subject: Death Notification.
